What does this form do?

When you use our services, we collect personal information about you – such as your name, address or phone number. Our privacy notice explains how and why we use this information – including what details we hold, who we might share it with, and your choices and rights. It also includes information about any checks and decisions we may make.

You can fill out this form to request a copy of the personal data we have stored about you. This is known as a Data Subject Access Request (DSAR).

We’ll send you a copy of the relevant data you’ve requested within 1 month. You can find out more about DSARs on the ICO website.

We’ll need:

• Your email address
• Your full name, address and date of birth
• A description of the information you’re requesting
